General Support Requests:  Assistance with applications, access requests, change requests etc.

Please include as much detail as possible so we can best provide service.

Support Request

 

New Employee Onboarding: To be submitted by the hiring manager. This will provide your new employee with access to Google Workspace (email) The Hive and Slack.

Be sure to note in the comments section any additional access that will be needed.  Form should be submitted at least 7 days before employee start date.

 

Onboarding

Employee Offboarding:  To be submitted by manager when an employee has left or will be leaving the organization.  This will revoke access to Google Workspace (email), Slack and The Hive. 

Please note in the comments any other applications that need to be revoked as well as who should receive access to the applicable files.
